New ADMC Request Form
Chapter Nine of the DME MAC Jurisdiction C Supplier Manual includes a section covering policies and procedures related to Advance Determination of Medicare Coverage (ADMC) for Wheelchairs. Included in the documentation requirements is a list of demographic information that the supplier must provide with the ADMC request. The manual states that failure to provide the required demographic data will result in rejection of the request.
CIGNA Government Services, in an effort to aid supplier compliance with the above requirement, has developed a form that suppliers can complete and attach to the front of their ADMC requests. Use of this form is recommended but is not mandatory. However, the first page of any ADMC request should clearly identify it as such.
