Gonadotropin Releasing Hormones for Prostate Cancer
Idaho LCD#2004-04

Indicate the 84-day depot injection of either agent by using “3” as the number of units of service. Providers should use the “GA” modifier to indicate that the Advanced Beneficiary Notice is on file. With the “GA” modifier, the provider can collect the co-payment on the Medicare allowance for the least costly medication and the difference between the submitted charge and the Medicare allowance for the least costly medication (not to exceed the difference in Medicare allowance between the two medications).

The current CMS Advanced Beneficiary Notice (ABN) should be used.
