Progressive Corrective Action Review - 11721
Progressive Corrective Action (PCA) was developed by the Centers for Medicare & Medicaid Services (CMS) to conduct medical review through sampled claims to validate potential errors and to educate providers concerning the errors. The goal of PCA is to lower the error rate. PCA probe reviews may be conducted either on a pre-payment or post payment basis.
A widespread post payment probe review was completed in August 2004 in Idaho for CPT code 11721*-Debridement of nail(s) by any method(s) six or more.
One hundred (100) claims were reviewed for this probe review. Twenty (20) providers were selected based on their utilization of CPT codes 11721*. The calculated error rate for this probe review was 5.50%.
The predominant error identified in this probe was the lack of documentation of complications of mycotic nails. The patient must have complications of the mycotic nail(s) that causes pain and/or infection and/or limitation of ambulation. This must be documented in the medical record.
