Erythropoietin Analogs For Non-Esrd Use (Epoetin Alfa And Darbepoetin Alfa) (L22203) - LCD

The following LCD's have been retired for Idaho: Darbepoetin and Erythropoetin. These policies have been replaced with Erythropoetin Analogs for Non-ESRD Use. Price Increase for R0070 and R0075

Price increases were made for codes R0070 and R0075 in Idaho only. The new payment rate is $109.13 and was entered into the CGS system on May 15, 2006. The increase will be retroactive back to January 1, 2006. A mass adjustment will be performed without having providers resubmitting claims.
