Revision of Erythropoietin Analog policy
The LCD for Erythropoietin Analogues for the three CIGNA Government Services states – Idaho, North Carolina and Tennessee – have been revised. Two ICD-9 codes, 285.21, anemia in chronic kidney disease, and V58.11, encounter for antineoplastic chemotherapy, have been added back to the policy as covered indications. Please note that these diagnosis codes must be billed with a secondary ICD-9 code. Please refer to the LCD for complete explanation.
